Monthly support group: provides one-on-one counseling as well as group classes to assist diabetic patients in self-managing their diabetes. The program is based on American Diabetes Association recommendations and guidelines.

* Home Health: intermittent "skilled" care in home for those who are homebound but do not need 24-hour professional care.
* Services: Nursing, physical therapy, social work, occupational therapy, and home health. Aides work with patients and families to manage acute, chronic and terminal conditions.
* Volunteer Hospice: respite and companion services provided by trained volunteers for terminally ill patients and families
* Lifeline: installation of emergency call system unit for home safety
